If there is not an actual leak in the pump, the pressure could be maintained for awhile without actual flow. Sounds weird maybe, but its a real life scenario.

As I've been fighting this problem in my own truck, I've found that the most common way for air to get into the fuel, at least on a 6.9 motor, is for the fuel return lines to crack. Start by replacing the main return line from injector #1 back to the fuel filter. You'll want 3/16 fuel line for that. After about 4 or 5 cycles the pump duration seemed to shorten up indicating to me there was less air in the line. After about 10 cycles I cranked the engine and it started kinda rough.

Close bleeder. Pump again until firm. This ^ 2nd round pressurizes fuel forward to the CP3. Go start it.
